Scholarship Requirements & FAQs
We know navigating a scholarship application can feel overwhelming, and we want to make it easier. This page answers the questions we hear most often from families, including who qualifies, what documents you will need, and how funding works. If you do not see your question here, we are always happy to help.
Our scholarship is open to K-12 students in Arizona whose families meet income or government assistance guidelines. Review the three eligibility criteria below, then scroll down for answers to the most common questions we hear from families before they apply.
Who Is This Scholarship For?
Enrolled in K-12 in Arizona
Your child must be currently enrolled in kindergarten through 12th grade in the state of Arizona. We will ask for a recent report card or school document to verify enrollment.
Meets Income Guidelines
Household income must be at or below 150% of the federal poverty guidelines based on your family size. Not sure if you qualify? Click here to check the current guidelines before you apply.
Receives Government Assistance
If your family receives government assistance such as SNAP, TANF, or SSI, you may also qualify. If you receive assistance, bring documentation that includes your benefit details and renewal or end dates.

You will be asked to upload the following:
Income documents (tax return or 3 months of pay stubs for each parent/guardian)
School verification (report card or official school document)
Government assistance documentation (if applicable, must include start and end/renewal dates)
If financial responsibility is clearly documented, such as the child being listed as a dependent on a tax return or included on government assistance, documentation from the other parent or guardian may not be required.
We require families to submit documentation verifying they meet our financial eligibility requirements (household income at or below 150% of the Federal Poverty Line).

Payments are sent directly to the activity provider on behalf of your child.
You will be required to provide the name, phone number, and email of the activity contact responsible for fee payments
Funds are not sent to families, and we do not offer reimbursements
Equipment and uniforms are shipped to your home
Providers must operate independently from the applying family
Do not pay the provider unless instructed to do so. If a partial scholarship is offered, you may be responsible for the remaining balance.

Applications are reviewed once all required documents have been received and verified
Decisions are typically made within 3 to 4 weeks of that confirmation
Missing or incomplete documentation will delay the process, so we recommend applying as early as possible
We are not able to offer same-day or expedited approvals
Scholarship FAQ

Opportunity4Kids provides scholarships for extracurricular activities that take place outside of the regular school day. This includes activities offered by schools or community organizations, whether on-campus or off-campus, as long as they are not part of required school coursework.
Activities such as sports, dance, arts, music, and other out-of-school programs may qualify.

Opportunity4Kids provides funds for:
Program or registration fees
Required uniforms
Required equipment
We do not provide funds for transportation, travel, or optional expenses associated with the activity.
All funds are sent directly to the activity provider or uniform/equipment retailer. Funds are not sent to families, and we do not offer reimbursements.

To qualify for a scholarship:
The child must be enrolled in K–12 in Arizona
Household income must fall within program guidelines based on household size
If you are unsure whether you qualify, you are encouraged to apply.

Household Income Verification:
Submit a tax return (IRS Form 1040) or three months of pay stubs for each parent/guardian. If one parent/guardian is not working, provide the most recent tax return.
If parents/guardians live in separate households, both must submit income documentation. Include any child support or custody agreements that outline financial responsibility.
If financial responsibility is clearly documented (such as the child being listed as a dependent on a tax return or included on government assistance), additional documentation from the other parent/guardian may not be required.
K–12 Enrollment Verification:
Submit the child’s most recent report card or another official school document that clearly shows both the child’s name and the school name.
For Parents/Guardians Who Do Not Work:
Provide a list of all government assistance received (such as SNAP, TANF, SSI, etc.), along with documentation showing start and end or renewal dates.
You must also include financial details demonstrating your ability to cover transportation and any incidental costs associated with participation.
AHCCCS documents are not accepted.
Please gather all required documents before starting your application.
If only one parent/guardian is completing the application and financial responsibility is not otherwise documented, the other parent/guardian may submit their documents by email to Scholarship@Opportunity4Kids.org with the child’s name in the subject line.

For program fees, Opportunity4Kids sends payment directly to the activity provider via check.
Equipment and uniforms are purchased online and shipped directly to you. Please use Amazon to identify the required items when completing your application.
Equipment
Required equipment may be requested on the application in addition to program fees. You will be asked to list each required item, including size, color, and any relevant details.
Uniforms
Required uniforms may also be requested on the application. If uniforms are purchased through the activity provider, the cost will be included in the fee payment.
If uniforms are purchased separately, please provide links along with size, color, and any other details needed to ensure correct items are ordered (such as brand, style, or team requirements).
All submitted items will be reviewed and verified before purchase.
Funds are not sent to families and we do not offer reimbursements.

Scholarship decisions are made after all required documents are received and reviewed.
We are not able to offer same-day or expedited approvals. Applications are reviewed in the order they are received.

Most decisions are made within a few weeks after your application has completed our initial staff review phase.
Incomplete applications may delay the review process.

Scholarship amounts vary by activity and available funding. In some cases, a partial scholarship may be offered.
Most scholarships average around $100 per month. Equipment and uniform support vary by activity and the expected use and longevity of the items.
